Brief Description

Please be aware that the Maryland Department of Health Survivorship Care Plan Enhancement Project Grant to Cancer Centers is being re-posted today 1.13.2020.  

As a change from the prior posting, we will be offering 1 grant of $100,000. We hope this will generate more interest. Deadline for submission is 1/27/2020.

In brief, the grant provides $100,000 to enhance current survivorship cancer plan activities and navigation. It is recognized that many cancer centers may already be doing some of the activities requested. The intent of the grant is to have the awardee expand current practices around survivorship care plan and support given to patients. Scope of work includes:

1) Provide a SCP to all patients who complete curative treatment and their referring physician and primary care physician

2) Assist patients in receiving appropriate follow-up care to facilitate timely access to quality health and psychosocial care as well as preventative care community/clinical linkages to services such as tobacco cessation, nutrition, and improved physical activity, as appropriate

3) Use, promote, and disseminate resources sponsored by the National Cancer Survivorship Resource Center increase cancer survivor and physician knowledge of guidelines for follow-up care. Awardees will utilize the ten existing e-learning series modules  developed for physicians by the National Cancer Survivorship Resource Center to educate their physician communities on care and follow-up guidelines for specific cancers. 

4) Evaluate the impact of the project and provide a written report that describes the development and impact of this project to furthering survivor care.
